–A primary care provider who contracts with a care time shall provide a care plan to the
care team for each participant, provide access to their health records, and meet
regularly with the care team to ensure integration of care.
–The HHS Secretary shall establish a program to implement medication management by
licensed pharmacists for those who take 4 or more prescribed medications
(including OTC medications and dietary supplements), take any high-risk medications,
have 2 or more chronic diseases, or have undergone a transition of care. The
services include assessing health and functional status, formulating a medication
treatment plan, and monitoring the progress.
–The HHS Secretary shall award a contract or grant to provide an emergency medical
and trauma system that coordinates all services within a region, ensuring
that a patient is taken in a timely fashion to the most appropriate facility,
allows for the tracking of hospital resources, includes a region-wide interfacility
data management system. The federal government will provide $3 for each $1 the
State provides. In addition, 100% of uncompensated care will be reimbursed for
Category A trauma centers, and 75% of uncompensated care will be reimbursed
for Category B trauma centers.
